Long Term Care Insurance

Client Responsibility:

	Call insurance and prequalify (if necessary).
	Let insurance company know agency name and give permission to obtain info.
	Obtain a claim form from insurance company.
	Fill out client portion of claim form.
	Have primary care physician fill out physician portion of claim form.
	Gather policy, policy number, & date of insurance eligibility for RN to view upon initial assessment.
	Schedule a visit from insurance rep to qualify for benefits (if required).
	Give Kelly’s Angels portion of agency claim form to fill out along with client & physician portion.
	Sign an Assignment of Benefits Form to allow Kelly’s Angels to receive payment directly from LTC insurance.
	Schedule an initial assessment to have Kelly’s Angels RN go over all details of the case and assess all needs.
	Let Kelly’s Angels handle the rest.

Kelly’s Angels Responsibility:

	Gather necessary insurance info from client/contact person during initial assessment.
	Call insurance to initiate case.
	Obtain a claim form for client if necessary.
	Submit physician claim portion to primary care physician.
	Submit medical plan of treatment to primary care physician.
	Obtain additional benefit coverage that client may not be aware of.
	File and send in initial benefit packet.
	Submit all billing statements and paperwork two times per month.
	Assist in any insurance calls, questions, resolutions so client does not have to.
	Receive benefit updates on a yearly basis (if applicable).
